Goal was to complete the SoDoH Screening Tool. It was completed. Provider is struggling because her housing is very unstable due to her oldest gchild’s and his father being homeless and she does not have the money to feed them. Will give her information on local food banks. Her car needs work and she is worried that something is going to happen to it and she uses it to transport her gchildren to school and their activities. She reports that her level of stress is very high and she is not sleeping. The relationship with her oldest son and his son is very conflictual right now and she struggles with getting her gson to comply with her requests. “How can I give anything to these children when I am so stressed that I can’t function?” She has been in contact with CPS about her oldest gson.
