Provider participated in health fair and had their blood pressure and sugar tested. Provider stated that their sugar was low. Provider usually checks their sugar in the morning and stated that the ate and took their meds, so was surprised at the number. Suggested to provider that their routine has been changed due to the shelter placement of the child. Provider agreed and was grateful for the fair and the results of the survey presentation. Was able to meet with the child and the child played with the other kid sand asked about getting a snack. Seemed happy and healthy.
