Consultation with Provider and the latest update with the child’s paternal grandmother (PGM). They had to be hospitalized because the oxygen machine that they were using was not helping to keep he O2 levels high and they were down to levels of 50. Shared that the family has been very stressed out and trying to make a plan that will allow for the PGM to go home with a 24 in-home care that will be provided by all of the PGM’s adult children. Provider wished there were more that she could do, encouraged that continuously providing care to the children is very helpful as child’s mother is doing a lot to assist with PGM. |
